The Asche operation consists of making two bisecting incisions midway of the greatest convexity of the septum, then bending the four points of cartilage thus left toward the concave side, and inserting in both nares either an Asche, or Mayer's modification of Asche's, hollow nasal splint. The after-treatment, during the four weeks in which the tubes are kept in the nares, consists of an occasional removal of the tubes and irrigation of the nose. While with this operation we have not yet reached the ultima thule of surgical correction of septal deviation, still for those cases of more or less complete stenosis due to deviation of the cartilaginous septum, it holds out relief which can be fairly safely promised our patient. The Asche operation has not yet received much vogue, for indeed no operation for a similar purpose has, most rhinologists contenting themselves with simply paring off the thickened portion of the septum, but being recently afforded the opportunity of seeing a number of cases upon which this operation had been done by Dr. Asche, I am convinced that it holds out the most practical and best results which have thus far been obtained by any operation of this character.
